            grade and every 10 years.

          _____ DPT-booster due after 4^th birthday

          _____ OPV/IPV/POLIO- booster due after 4^th birthday

          _____ MMR- (Measles, Mumps, Rubella)-1^st or 2^nd dose

          _____ HIB

          _____ Hepatitis B- three doses required

_____ Varicella (chicken pox) vaccine 1^st or 2^nd dose or written        

           validation of chicken pox illness (month & year)

_____ Pneumococcal Conjugate (4 doses required for PRE-K)

_____ Hepatitis A (series of 2 doses, required for PRE-K & K)

_____ Meningococcal required for entry into 7^th grade

_____ HPV (series of 3 shots) 

Students can be kept from attending school if they do not have

the required immunizations.

The Corsicana/Navarro County Health Department (903-874-6731) gives shots everyTUESDAY AND WEDNESDAY for those who do not have insurance that covers immunizations.

YOU MUST SEND PROOF OF YOUR CHILD RECEIVNG THE IMMUNIZATIONS!
